Dental Hygiene Application Information

 

DENTAL APPLICATION INFORMATION

 

Examination Eligibility:  (Both the traditional and Pre-Graduation Examinations): The entire Pre-Graduation examination will be available to senior dental students, post-graduate dental students and residents, and dental school graduates based upon testing site availability.  Qualified dental students in participating schools who are in their Junior year are also eligible to take the manikin-based examination.  

In some instances there are more applicants for an examination than there are seats available at a test site.  When that occurs seating is assigned for both the Pre-Graduation and Traditional examinations in the following order of priority:  (1) eligible students of record enrolled in the undergraduate program at the Dental School at which the examination will be given; (2) eligible students of record enrolled in a graduate (post-graduate) program at the Dental School at which the examination will be given; (3)  eligible students of record enrolled in an undergraduate program at a Dental School located in another CITA Member State; (4) eligible students of record enrolled in a graduate (post graduate) program  at a Dental School located in another member CITA state (5) graduates who are citizens of a CITA member state (6) all other eligible students currently enrolled in dental school (7) all other dental school graduates. 

Application Policy:  Candidates taking the examination must apply and pay the Examination Fee not less than ninety (90) days before the examination being applied for.  Candidates who are unsuccessful with any of the Parts must reapply and pay the Retest Examination Fee(s) not less than thirty (30) days before the examination being applied for.

Examination Fee:  The examination fee is must be paid at the time of application.  The fee for the initial examination (Parts II, III, IV and V) is $1600..

Retest Examination Fee: Candidates applying for any retest examination will be required to pay $800.00 for each Session where the candidate has failed either one OR both Parts of the Session.  Candidates applying for any retest examination will be required to pay $800.00 for each Session where the candidate has failed either one OR both Parts of the Session.

An administrative fee of $100 is included in the above fees. This administrative fee is non-refundable and deducted from all returned application fees. Re-submissions of applications which were returned for any reason must contain an additional administrative fee of $100 to bring the resubmitted fee back to the base filing fee.

Under certain circumstances, such as moving a candidate to an alternative examination site to accommodate a candidate’s request or conflicts, the $100 administrative fee will also be imposed. In such cases the candidate will be notified accordingly.

The examination fee must be paid at the time of application. Payment submitted must be for the exact amount and be paid by cashier’s check, certified check or money order with the applicant’s social security number written in the lower left-hand corner.

PERSONAL CHECKS WILL NOT BE ACCEPTED AND WILL BE RETURNED TOGETHER WITH THE APPLICATION TO THE APPLICANT. The testing agency is not responsible for a missed deadline date if an application is returned.

 Dental Examination Application

The exam application is a PDF file (Adobe Acrobat) that can be downloaded and completed using the computer keyboard or can be downloaded, printed and completed by hand.  Applications are scanned into the CITA database; therefore, it is IMPERATIVE that you follow the application instructions and fill out the application correctly.

HYGIENE APPLICATION INFORMATION

Facility Fee:  A facility fee may be charged by the testing site for a site/rental fee for use of instruments, clinic facilities, supplies and disposables.  Some schools require that all instruments be supplied by the testing site school.  A rental charge or deposit imposed by the school must be remitted directly to the school.

Retake Examination Fee: Candidates who are unsuccessful with any part of the examination must submit a new application and pay  a $750 retest fee.
